Instead of sending a single request for all the activities, fetch all of them based on phases they belong in parallel

PlanActivities: Make each phase independent component

This commit add alot of features as follow

- All plan phase activities will load in parallel independent of
each other
- Update of plan activity will reload on changed phase an not all phases
- Adding new activity will only reload updated phase list and not all
phases
- Phases header now have numbers that reflects the count of results
in the list

This commit close #125
