A common workflow is to make a metareview in one comment that should be eventually visible to the authors, and then to make another comment asking the reviewers to check the metareview. The latter comment is only intended for the reviewers, and may indeed contain confidential information. But once one has selected eventually visible to the authors, that is the default for the messages that come after it, which may be easy to overlook.
It would be better if the default was always to the reviewers only. Forgetting to give out information can be corrected, but giving out too much information cannot.
