Right now, the process of adding a toilet is very confusing. Is adding a separate marker always necessary? Can a toilet be marked as a part of or within a building?
There is already a field to mark wheelchair access, but the gender designation tag must be marked manually.

Sounds good.. You can see the OSM wiki recommended tagging for toilets. These are normally added as a point on the map where the restroom is.
There are some gender designation preset fields from that wiki that we could add to iD (male/female/unisex).
If you have an idea for an improved tagging scheme that might make it easier for your project to catalog gender-friendly restrooms, you could discuss it on the tagging mailing list. That is the generally accepted way of raising awareness of a new tag that you want to introduce. Good luck!
