Cannot upload proof

Hello everyone,
I am trying to apply for GitHub student benefits again. But, there is some weird thing going on. When I tried on my PC which has no web-camera, I couldn't upload a file. Only gave me the option to take a photo and then it bugged out.
When I tried it on my phone, I could only use my Front Camera, if I try to use my back camera, it just white screens and I can't take a picture. With the front camera, I am telling you, I tried every single angle possible, but aint no way I get a picture where you can read everything.

How can I fix this and upload the document I intend to?
