refactor: update to remix vite

• closes refactor: use remix unstable vite #518

Basic idea is:

• grab import * as build from "virtual:server-entry directly and use with createRequestHandler like before (and use as hattip handler)
  • TODO: maybe it's possible to still reuse remix-vite middleware by prepending our own middleware.
    • probably not. next call is not wrapped by async context https://github.com/hattipjs/hattip/blob/37f824115c85ef96f6e2b28cfe289f882f225c73/packages/adapter/adapter-node/src/common.ts#L93-L98
• ignore remix's default middleware completely and handle request from own handler via @vavite/connect
  • TODO: is explicit moduleGraph.invalidateModule necessary for virtual modules on ssr? this is fine
  • instead of vavite, created own minimal plugin feat: create vite-plugin-ssr-middleware vite-plugins#116

• fix uno.css infinite update (probably same issue as (reproduction) chore: infinite hot unocss reproduction vite-plugins#12)
  • I still don't know why but moving import "virtual:uno.css" from entry.client.tsx to root.tsx fixed it
• fix FOUC on dev (same idea should work feat(demo): create vitePluginSsrCss vite-plugins#110)
