The index disappeared when we moved to the readthedocs theme. To fix it I added a dummy file genindex.rst and added genindex.rst to the index.rst file. This is clearly a hack but it works.

It appears that generation and the web page tags pointing to the documentation index have completely disappeared. My first guess would be that this happened when we changed the theme from sphinxcontrib.fulltoc to sphinx_rtd_theme.
